Adrenal Fatigue (Kidney Qi Deficiency)

shènxū píláo · Adrenal Fatigue (Kidney Qi Deficiency)wellness

Overview

Adrenal fatigue corresponds to the categories of 'consumptive exhaustion' (xulao) and 'lassitude' (juandai) in TCM, with kidney qi deficiency as the core pathogenesis. It commonly arises from congenital insufficiency, chronic illness, overwork, or age-related kidney decline. The kidney stores essence, governs bones and marrow, and is the root of original qi (yuanqi). When kidney qi is insufficient, essence fails to transform into qi, leading to depleted original qi. Manifestations include mental fatigue, weakness of the lower back and knees, dizziness, tinnitus, memory decline, aversion to cold with cold limbs, frequent nocturia, and decreased libido. The tongue is pale and swollen with a white coating; the pulse is deep, thin, and weak, especially at the chi position. This condition is fundamentally deficient, often complicated by phlegm-dampness, blood stasis, or liver qi stagnation, forming a mixed pattern of deficiency and excess.

Pattern differentiation (clinical patterns)

Kidney Qi Deficiency Pattern

Clinical presentation

Mental fatigue and weakness, soreness and weakness of the lower back and knees, dizziness and tinnitus, memory decline, frequent nocturia, decreased libido, pale swollen tongue with white coating, deep thin weak pulse, especially at the chi position.

Treatment principle

Tonify kidney qi, fill essence, and nourish the spirit.

Kidney Yang Decline Pattern

Clinical presentation

Aversion to cold with cold limbs, cold and painful lower back and knees, listlessness, pale complexion, frequent nocturia or clear profuse urine, loose stools, decreased libido, pale swollen tongue with teeth marks and white slippery coating, deep slow weak pulse.

Treatment principle

Warm and tonify kidney yang, disperse cold, and consolidate the root.

Kidney Essence Insufficiency Pattern

Clinical presentation

Emaciation, dizziness and vertigo, tinnitus and hearing loss, soreness and weakness of the lower back and knees, forgetfulness and mental fog, hair loss and loose teeth, low sperm count or infertility in men, scanty menstruation or amenorrhea in women, pale red tongue with little coating, thin weak pulse.

Treatment principle

Tonify the kidney and fill essence, nourish yin and blood.

Daily care & prevention

Maintain a regular sleep schedule, avoid staying up late and overexertion, and ensure adequate rest. Diet may include black sesame, walnut, Chinese yam, goji berry, and mutton to tonify the kidney and fill essence; minimize cold and raw foods. Practice moderate exercise such as Baduanjin or Taijiquan to cultivate original qi, avoiding strenuous activities that consume qi. Keep the lower back and abdomen warm; moxibustion at Guanyuan (CV4) and Shenshu (BL23) for 10-15 minutes daily is beneficial. Regulate emotions and reduce mental stress.

When to seek medical care

Seek immediate medical attention if there is persistent high fever, rapid weight loss, severe anemia, palpitations with chest pain, confusion, oliguria with edema, or marked blood pressure abnormalities, to rule out organic disease. Fatigue accompanied by unexplained lymphadenopathy, night sweats, or bleeding tendencies warrants prompt investigation for malignancy.
