Ankylosing Spondylitis (Bone Bi)

gǔbì · Ankylosing Spondylitis (Bone Bi)rheum

Overview

Bone Bi falls within the category of Bi (impediment) disorders in TCM. Its main manifestations are pain and stiffness of the lumbosacral region and spine, with difficulty in bending and extending, and in severe cases, spinal rigidity and deformity. Ankylosing spondylitis is mostly caused by congenital insufficiency, deficiency of the kidney and Du vessel, and invasion of wind, cold, dampness, and heat pathogens into the bone joints, leading to phlegm and blood stasis binding together and obstructing the Du vessel. The disease is located in the bones and Du vessel, involving the kidney, liver, and spleen. Common patterns include kidney deficiency with Du cold, damp-heat impediment, and phlegm-stasis obstructing the collaterals. Early stages are dominated by excess pathogens, while late stages show a mixture of deficiency and excess, with root deficiency and branch excess.

Pattern differentiation (clinical patterns)

Kidney Deficiency with Du Cold

Clinical presentation

Cold pain in the lumbosacral region, spinal stiffness aggravated by cold and relieved by warmth, aversion to cold with cold limbs, soreness and weakness of the lower back and knees, frequent nocturia, pale tongue with white coating, and a deep fine pulse.

Treatment principle

Warm the kidney and strengthen the Du vessel; disperse cold and unblock impediment.

Damp-Heat Impediment

Clinical presentation

Burning pain in the lumbosacral region, stiffness with tenderness, aggravation by heat, joint swelling, heaviness and fatigue of the body, bitter sticky taste in the mouth, yellow and dark urine, red tongue with yellow greasy coating, and a slippery rapid pulse.

Treatment principle

Clear heat and drain dampness; unblock the collaterals and relieve pain.

Phlegm-Stasis Obstructing the Collaterals

Clinical presentation

Stabbing pain in the lumbar spine with fixed location, stiffness and deformity, restricted movement, dark complexion, dry skin, purple-dark tongue with ecchymosis and white greasy coating, and a choppy pulse.

Treatment principle

Transform phlegm and dispel stasis; unblock the collaterals and strengthen the Du vessel.

Daily care & prevention

Keep the lower back and spine warm, and avoid prolonged sitting on damp ground or in cold-damp environments. Maintain regular functional exercises such as Ba Duan Jin, Tai Chi, and swimming to preserve spinal mobility. Sleep on a firm mattress with a low pillow, and maintain correct sitting and lying posture. Diet should be warm and tonifying; appropriately consume kidney-tonifying and Du-strengthening foods such as black beans, walnuts, and Eucommia stewed pork kidney. Quit smoking, regulate emotions, and avoid overexertion.

When to seek medical care

Sudden severe low back pain with fever, numbness and weakness of the lower limbs, incontinence or retention of urine and stool suggests cauda equina syndrome or infection, requiring immediate medical attention. Rapidly worsening spinal deformity, markedly decreased chest expansion, recurrent iritis, or cardiac conduction abnormalities warrant timely specialist evaluation.
