Rheumatoid Arthritis & Bi Syndrome (Joint Pain)

bìzhèng · Rheumatoid Arthritis & Bi Syndrome (Joint Pain)Pain & Musculoskeletal

Overview

Rheumatoid arthritis is an autoimmune disease characterized by symmetric, erosive swelling and pain of small hand/foot joints, morning stiffness often over one hour, and deformity, with possible systemic involvement. Chinese medicine places it under "bi syndrome," caused by wind, cold, damp, or heat pathogens obstructing channels and impeding qi–blood flow; patterns include wind-cold-damp bi, wind-damp-heat bi, phlegm-stasis obstruction, and liver–kidney deficiency. It requires standard rheumatology treatment with disease-modifying drugs; TCM is adjunctive, not a replacement.

Pattern differentiation (clinical patterns)

Wind-Cold-Damp Bi

Clinical presentation

Cold painful joints with difficult flexion/extension, fixed or migrating pain worse with cold and relieved by warmth, morning stiffness, white greasy coating, wiry tight pulse.

Treatment principle

Dispel wind and cold; eliminate dampness and unblock collaterals.

Wind-Damp-Heat Bi

Clinical presentation

Red, hot, swollen, exquisitely tender joints slightly relieved by coolness, fever, thirst, irritability, red tongue yellow greasy coating, rolling rapid pulse.

Treatment principle

Clear heat and unblock collaterals; dispel wind and drain dampness.

Liver–Kidney Deficiency

Clinical presentation

Long-standing bi with deformed stiff joints and muscle wasting, weak low back/knees, dizziness, fatigue, pale tongue, fine weak pulse.

Treatment principle

Tonify liver and kidney; strengthen bones and sinews.

Daily care & prevention

Take disease-modifying antirheumatic drugs regularly as prescribed by rheumatology—do not stop them or rely solely on folk remedies; rest during flares and do moderate activity and strength training in remission to preserve function, keep warm, stop smoking, and care for teeth (reduce infection). Gently massage/moxa Zusanli (ST36), Yanglingquan (GB34), Shenshu (BL23), and local tender points. Sudden hot swollen joints with fever, chest symptoms, or rash needs prompt care.

When to seek medical care

Sudden hot red swollen joint(s) with fever/chills (septic arthritis or acute gout) needs urgent treatment; chest pain/tightness, breathlessness, leg edema, or fever/infection while on steroids/immunosuppressants requires prompt care.
