Vulvar & Vaginal Itching

Also called:Vaginal itchingVulvar itchingGenital itch
From the TCM perspective: TCM 'yin yang' (genital itch); located in the liver channel and Chong-Ren at the anterior yin, involving spleen and kidney.

Overview

Vaginal/genital itching is itching, burning or irritation of the vulva and vaginal area. TCM groups it under 'genital itch' and 'discharge disorders'; the main mechanisms are damp-heat pouring downward, damp-toxin (parasitic) erosion, and liver-kidney yin deficiency failing to nourish, while external wind-heat/contact allergy and infections must also be excluded.

☯Common patterns (syndrome differentiation)

1

Damp-heat pouring downward

Key distinguishing signs
Genital itching with copious thick yellow malodorous discharge, bitter taste, dark urine, red tongue with yellow greasy coat, slippery rapid pulse.
Treatment principle
Clear heat, drain dampness and stop itch
2

Damp-toxin / parasitic erosion

Key distinguishing signs
Intense itching with gray-white, curd-like or frothy discharge and marked odor, burning pain, red tongue with yellow coat, rapid pulse.
Treatment principle
Clear heat, resolve toxin, dry dampness and address parasites
3

Liver-kidney yin deficiency

Key distinguishing signs
Dry genital itching worse at night with scant discharge, heat in palms/soles, dizziness, tinnitus, sore weak knees, red tongue with little coat, fine rapid pulse.
Treatment principle
Nourish liver-kidney, moisten dryness and stop itch
4

External wind-heat / contact allergy

Key distinguishing signs
Sudden itching with red swollen vulva related to a new sanitary product, wash, underwear fabric or medication, possibly with rash, thin coat, floating pulse.
Treatment principle
Dispel wind-heat and remove the trigger

Daily self-care

  • Keep the vulva clean and dry in breathable cotton underwear; avoid douching, scented products and tight non-breathable clothing.
  • Avoid scratching and hot washing; stop possible triggers (new products, intercourse) and follow the prescribed course of treatment.
  • Itching with abnormal discharge/odor or recurrence warrants discharge testing and cause-specific treatment rather than prolonged self-use of antibiotics or steroid creams.

When to seek medical help

The following warning signs require prompt medical evaluation — do not rely on self-treatment:

  • Genital itching or abnormal discharge during pregnancy: seek early obstetric assessment for safe treatment.
  • Fever, lower-abdominal pain or purulent foul discharge: possible pelvic infection — prompt care.
  • Genital itching, abnormal discharge or injury in a young girl: timely pediatric/gynecologic examination.
