From the TCM perspective: Restless legs: damp-heat or blood/yin deficiency in the lower collaterals
Overview
Restless legs is an overwhelming evening urge to move the legs with tingling and tension, relieved by movement and disrupting sleep. TCM sees qi-blood stagnation in lower-limb collaterals, either from damp-heat pouring downward or from blood/yin/essence deficiency depriving collaterals of nourishment.
☯Common patterns (syndrome differentiation)
1
Blood and essence deficiency
Key distinguishing signs
Evening restlessness with dizziness, tinnitus, dryness and cramps, weak lower back, pale tongue
Treatment principle
Nourish blood/essence and relax the tendons
2
Damp-heat pouring downward
Key distinguishing signs
Heaviness and warmth in the legs, sticky bitter mouth, dark urine, yellow greasy coat
Treatment principle
Clear heat and drain dampness
3
Blood stasis in collaterals
Key distinguishing signs
Fixed pain/tingling, prominent veins, dusky color, improved by massage and movement
Treatment principle
Quicken blood and remove stasis
4
Insufficient yang reaching the limbs
Key distinguishing signs
Cold restless legs, fear of cold and fatigue, worse at night, pale moist tongue and deep slow pulse
Treatment principle
Warm yang and unblock the collaterals
Daily self-care
Keep regular sleep, stretch/massage the legs and take a short evening walk.
Avoid evening caffeine/alcohol and check iron, B12 and folate as advised.
Take a warm bath, use compresses and avoid prolonged immobility.
When to seek medical help

Weakness or ascending numbness, or loss of leg reflexes.
Symptoms after injury, or swelling/redness/warmth in one leg (possible DVT).
Severe ongoing insomnia with dangerous daytime sleepiness or depression.
