Postpartum hair loss

Also called:postpartum hair sheddingtelogen effluvium postpartum
From the TCM perspective: Postpartum shedding: blood/yin deficiency leaving the scalp undernourished

Overview

About 2-5 months after birth many women shed heavily as hair cycles normalize after pregnancy; it is mostly temporary and improves in 6-12 months. TCM sees pregnancy and breastfeeding depleting blood, qi and essence so the hair is undernourished, sometimes with scalp damp-heat. Persistent patchy loss needs separate evaluation.

☯Common patterns (syndrome differentiation)

1

Blood deficiency not nourishing hair

Key distinguishing signs
Diffuse dry shedding with dizziness, pallor and poor sleep, pale tongue and fine pulse
Treatment principle
Nourish blood to moisten the scalp
2

Liver-kidney essence deficiency

Key distinguishing signs
Shedding with premature graying, tinnitus and weak lower back-knees, especially with long breastfeeding
Treatment principle
Tonify liver-kidney essence
3

Blood heat with wind-dryness at the scalp

Key distinguishing signs
Red itchy scaly scalp with hair becoming oily quickly and uneven shedding
Treatment principle
Cool blood and moisten dryness
4

Spleen deficiency with dampness

Key distinguishing signs
Oily matted hair with fatigue, bloating, loose stool and poor appetite, greasy tongue coat
Treatment principle
Strengthen the spleen and drain dampness

Daily self-care

  • Expect recovery; eat balanced protein/iron/biotin, rest and support yourself while breastfeeding.
  • Comb gently, avoid tight styles, high heat and dyes; a shorter cut reduces the appearance of thin ends.
  • Do not stack supplements/herbs while breastfeeding without a clinician; check iron and thyroid if shedding is severe and prolonged.

When to seek medical help

The following warning signs require prompt medical evaluation — do not rely on self-treatment:

  • Distinct round bald patches or shedding with scarring/redness/intense itch.
  • Rapid generalized loss with marked fatigue, coldness or weight change (thyroid/iron).
  • Shedding after severe fever/complicated delivery with hemorrhage, or severe low mood.
